SUD Housing Assistance Frequently Asked Questions (FAQ):
Q: Who is eligible?
A: Idaho residents who are currently taking part in SUD treatment services or have successfully completed SUD treatment within the last 90 days and meet income eligibility requirements.
Q: What does the program cover?
A:This program covers rent or mortgage assistance and utility payments including power, water, sewer, gas, and trash. Eligible participants can access the program for one or all of these services. Reimbursement is provided directly to the landlord/mortgage company, or utility company.
Q: How long does the program last?
A:Eligible participants who continue to meet program requirements can receive up to 6 months or $8,500.00 of assistance, whichever limit is reached first.
Q: What information is needed to apply?
A: All the information needed is listed in the application. This includes some personally identifiable information. We use an online platform that meets HIPAA and 42 CFR security standards. Rent/mortgage and utility bills will be required, as payments are made directly to rent/mortgage and utilities companies.

About the Program:
The Provider Stabilization Grant Program (PSGP) is designed to provide financial support to providers of substance use disorder (SUD) and recovery support services (RSS) who have experienced financial hardship due to the COVID-19 pandemic. Applicants must demonstrate lost revenue and incurred specific expenses during the period of March 1, 2020, through December 31, 2021. Calculated payments will be reduced by funds received during the same period through any of the COVID relief programs.
Who is eligible to apply for this program?
Providers currently providing services funded by the Substance Abuse Treatment Block Grant are eligible for this grant program, which includes all providers currently in the BPA Health SUD network. Specific eligibility requirements apply as outlined in the application and Terms and Conditions.
How do I qualify?
Potentially eligible providers must submit a complete application including:
- 2019-2021 tax returns (or 2019-2020 if 2021 tax return is not yet complete)
- 2019-2021 monthly financial statements
- Documentation of all supplemental COVID relief funds received
Eligibility criteria and restrictions are detailed in the Terms and Conditions. All application information is kept secure and confidential.
What is ‘lost revenue’ and how are payments calculated?
“Lost revenue” refers to revenue a provider lost due to COVID-19. This may include revenue losses associated with fewer visits, cancelled procedures/services, increased agency costs directly attributed to the COVID-19 pandemic or government-directed shutdowns.
Lost revenue will be estimated for the impact period (March 1, 2020 through December 31, 2021) by comparing total revenue from all sources in that period to an “average” month from the applicant’s historical baseline. Stabilization payments will be made of up to 33% of historical revenue, and not to exceed estimated revenue lost.
Grant requests will be individually evaluated against the program criteria. Grant applications may be partially or fully approved in accordance with program criteria. Once all applications have been evaluated, a total eligible request amount will be determined.

The goal of the Substance Use Disorder (SUD) COVID-19 Housing Assistance Program (SUDHAP-19) is to assist SUD households who have experienced financial hardships due to the COVID-19 pandemic. The program provides rent, mortgage, and utility assistance for up to six months or $8,500 for each qualifying household or individual, whichever comes first. Funding is available on a first come first serve basis.
To qualify for this assistance applicants must:
1. submit completed application and copies of rent/mortgage and/or utility statements;
2. have provider verify that the applicant is currently enrolled in and attending substance use disorder treatment OR attest the applicant has successfully completed treatment in the past 90 days;
3. while receiving assistance, the applicant must submit by the 15th of the month statements of housing and/or utility bill(s) requesting payment for; and
4. If applicant is still in treatment the provider must submit the attestation form each month prior to receiving continued SUDHAP-19 funding, if applicant has completed treatment the applicant must check-in with BPA Health by the 15th of each month.

This form is to be completed by providers who are submitting substance use disorder assessments (i.e. CDAs) other than GAINs in WITS for review and consideration of authorization of SUD funded services, including those not covered by Medicaid. In-network providers who have completed a GAIN should consent within WITS and NOT use this form.
Please be aware that your client must first call BPA Health at 1(800) 922-3406 and be screened for funding eligibility. Once funding eligibility has been met, please complete and submit this online form. Any forms submitted prior to funding approval will be denied and you will be required to re-submit a new form after the screening has been completed.

This document includes a general description of changes since the last manual update and the date of the change. They will be written into the next manual revision. List of updates can be found here.
BPA Health has received feedback from stand-alone RSS providers asking for help navigating the Provider Manual. In an effort to deliver information from the Provider Manual that is pertinent only to RSS providers, BPA Health has created the RSS Guide to the BPA Health Provider Manual.

All treatment providers and sites in the BPA Health SUD network must be co-occurring capable. Be prepared to demonstrate these capabilities in a co-occurring assessment form. This form is required to be submitted at time of facility renewal as well as any time a provider believes they may have moved from Capable to Enhanced.
