Peer and Family Support Certification Application

Greetings and glad you’re here! Certification for Idaho’s Peer Support Specialists and Family Support Partners is managed by BPA Health. Please see below for your application options. Please note you can save and return to an application that you start online if you aren’t able to complete it at one time. You can also upload your supporting documentation within your online application. If needed, you may request a paper application by emailing [email protected], however we highly encourage applicants and supervisors to use the online forms below to submit their applications.

 

INITIAL 6-MONTH CERTIFICATION

Apply for your 6-month Peer Support Specialist or Family Support Partner Certification

Request an extension of your 6-month certification to complete your supervision and/or work experience

 

SUPERVISOR VERIFICATION*

This is the form that must be submitted by supervisors for CPSSs/CFSPs who currently hold a 6-month certification and have completed their hours of supervision and work/volunteer experience.

CERTIFICATION RENEWAL

Renew your 1-year certification

Request an extension of your 1-year certification to complete your hours of continuing education

INACTIVE CERTIFICATION STATUS
