Wraparound Intensive Care Coordination

Wraparound is a family-driven, team-based process in Idaho’s Youth Empowerment Services (YES) that builds support networks to help youth confidently address complex challenges. This is accomplished by collaborating with the Wraparound Team to create and implement a personalized Plan of Care for the child or youth.
