Wraparound Intensive Services Wraparound is a family-driven, team-based process within Idaho’s YES system that helps youth and families get the support they need during challenging times. Funded by the State of Idaho, Wraparound services are available at no cost to families. Families work alongside a Wraparound team to create a personalized Plan of Care that connects them with the people and resources that can best support their child and family. What to Expect from Wraparound Services? Wraparound Care Coordinators meet regularly with families to check in on progress, talk through challenges, and adjust supports as needed. They help families access resources, advocate for their needs, and bring together schools, providers, and other services so families don’t have to navigate disconnected systems on their own. It brings together professional supports, like therapists and providers, with natural supports such as family members and trusted friends. The process focuses on building skills, strengthening relationships, and helping families create lasting stability and support. Youth and Children (18 and under) Wraparound supports youth and families who may need help coordinating care and navigating multiple challenges. The program is available to youth ages 18 and under. Youth who began receiving EPSDT services before turning 18 may continue participating in Wraparound until age 21. Youth With an Individual Needs Assessment Wraparound may be a good fit for youth who have completed a Child and Adolescent Needs and Strengths (CANS) assessment with an overall score of 2 or 3. If an assessment has not been completed yet, our Care Coordinators can help determine eligibility during the initial visit. Youth Involved in a Minimum of Two Areas of Support Behavioral health involvement and involvement in at least one additional child serving system: Court or juvenile justice programs Child protective services (CPS) Educational support (IEP, 504 plans, or a desire for academic improvement) Developmental disability or intellectually disabled services Physical health needs Youth Eligible for Medicaid in Idaho Many youth in Wraparound receive Medicaid. We work closely with caregivers, parents, guardians, and other supportive adults in a youth’s life who are willing to participate in the process and support the youth’s success. How is the family involved in the Wraparound process? Families play a central role in the Wraparound process. Their voices help guide decisions and shape the Plan of Care. Families help choose who is part of their team and work alongside them at every step to ensure the plan reflects their goals, values, and priorities. What kind of support can families expect from a Care Coordinator? Care Coordinators help guide the Wraparound process by organizing team meetings, supporting the development of the Plan of Care, and staying connected with the family and the team. They advocate for the family’s needs and help adjust supports as situations and needs change. How are team members selected for the Wraparound process? Youth and families help choose the members of their Wraparound team. This can include both formal supports, such as counselors, educators, or service providers, and informal supports, like extended family members, friends, or other trusted adults. The team is built around the youth and family to provide coordinated support that reflects their needs and goals. Team members are individuals who care about the youth’s well-being and are committed to supporting their success. What happens if the family’s needs change during the process? The Wraparound process is designed to be flexible. As needs change, the Plan of Care can be updated to reflect new challenges, goals, or strengths. During regular team meetings, families and team members work together to adjust supports so they continue to meet the youth and family’s needs. Respect for each family’s culture and background is an important part of the Wraparound process. The team works with the family to ensure the Plan of Care reflects their cultural values, traditions, and language needs. This helps make the support provided meaningful, respectful, and relevant to each family’s unique situation. Can families still participate in Wraparound if they have existing services in place? Yes. Wraparound is designed to work alongside existing services, not replace them. The process helps bring different supports together so everyone involved is working toward the same goals and supporting the family in a coordinated way. What does the term ‘family-driven, youth-guided’ mean in practice? “Family-driven, youth-guided” means that families and youth play a leading role in the Wraparound process. Their perspectives, experiences, and goals help guide team conversations and decisions, ensuring the Plan of Care reflects what matters most to them. Is Wraparound a Service or an Intervention? Wraparound is not a standalone service or an immediate crisis intervention. Instead, it is a planning and coordination process that brings together multiple services and supports to help meet the needs of youth and families.
